For Immunization Records, please utilize the Idaho Department of Health and Welfare’s Docket® application on your mobile device. The Docket® application will allow access to your records housed in Idaho’s Immunization Reminder Information System (IRIS).

Medical Records Requests

For Medical Records Requests (non-immunization) related to medical services performed at Southeastern Idaho Public Health, please fill out and submit a Personal Medical Records Request form: Personal Medical Records Request Form

To release your records to another medical provider, please fill out an Authorization for Release of Information Authorization for Release of Information or call your local SIPH Office.
