Clinical Services

Southeastern Idaho Public Health (SIPH) is dedicated to serving the communities of our 8 county district. We believe in providing non-judgmental, holistic care, that helps promote every client’s well-being and improves our client’s overall health. We empower all of our clients to find meaningful solutions to their healthcare needs. If we don’t offer a service that our clients need, we will link them to other community resources that can.

One of SIPH’s main priorities is to maintain confidentiality for all of our clients, including teens. We invite teens and adolescents to visit our offices if they need to discuss STI prevention, birth control options and other heatlh concerns with our caring providers.

We offer in-person office visits as well as telehealth visits for certain appointment types. Clinical Services are provided by appointment only. Same-day appointments may be available. Please call your local public health office.

Luis Fernandez, MD, Medical Director
Luis Fernandez, MD, Medical Director

  • Is board certified in family medicine. He graduated from Instituto Superior de Ciencias Medicas, Cuba and completed residency training at Idaho State University Family Medicine.
  • As a Family Medicine physician, Dr. Fernandez diagnoses and treats a wide array of ailments for family members both young and old. This multifaceted aspect of Family Medicine is one of the things he greatly enjoys. Every patient presents a new challenge. Dr. Fernandez also likes the fact that in addition to treating existing health problems he is able to focus his patient’s attention on preventive health care. Sharing with them things they can do to get healthy and stay healthy. His belief is it’s always better to “prevent than to prescribe.”
  • Dr. Fernandez practices what he preaches through outings with his wife and two children that include x-country and alpine skiing, biking and running.

Tina Mladenka, APRN, WHNP-BC, DNP
Tina Mladenka, APRN, WHNP-BC, DNP

  • As a Board Certified Women’s Health Nurse Practitioner, Tina Mladenka joined the SIPH Health Care Team in 1997. She has over 35 years of health care experience that includes reproductive health, prenatal care, gynecological care, and primary health care services, as well as RN experience in labor and delivery.
  • She is driven toward public health because of its opportunity to provide current, personalized health care services to clients, regardless of their income, and empower them with education to help them make beneficial choices regarding birth control, avoid and treat STDs, get cervical cancer and breast screening, and understand their bodies to stay healthy.
  • Tina appreciates the contributions that all members of the health care team provide to ensure quality patient care at SIPH.
  • She loves the mountains and intermountain west because of the beauty and numerous recreational opportunities she enjoys in her free time.

Ian Troesoyer, Nurse Practitioner
Ian Troesoyer, Nurse Practitioner

  • Ian Troesoyer is a board-certified Family Nurse Practitioner. He earned his Doctor of Nursing Practice from Idaho State University. Ian deeply enjoys spending time getting to know his patients and learning how he can best help them. He is drawn to public health for the opportunity to find answers to systemic challenges that affect the health of entire communities.
  • He feels honored to work for all the people of Southeastern Idaho, regardless of their ability to pay or otherwise access typical healthcare services.
  • He is passionate about preventive health, primary care, evidence-based medicine, and data science.
  • In his free time, Ian loves playing music, adventuring outside, and spending time with his wife and 2 kids.

Chris Cole, LPC, NCC
Chris Cole, LPC, NCC

  • A recent graduate of Idaho State University’s Master of Counseling program, Chris Cole started at SIPH as a counselor in 2023. He has worked in various industries throughout his life before becoming a counselor, including television news, education, and marketing.
  • Born and raised in Idaho, Chris has a passion for serving his community in various capacities, and is highly interested in helping underserved populations and minority-identifying individuals.
  • Chris is excited to be working in public health, as he firmly believes health care is a right which should be provided to everyone, regardless of income.
  • In his free time, Chris enjoys attending and participating in the local theater community, nonprofit work, devouring books, music, and movies, and spending time with his family, friends, and dogs.
