What does this program do?

Primary care services include:

  • Wellness screenings and preventive care
  • Diagnosis and treatment of common conditions such as diabetes, high blood pressure, high cholesterol, asthma, and COPD
  • Physical exams and routine health evaluations
  • Lab work, imaging orders, and prescriptions
  • Referrals to specialists when needed
